Business Rates customers can use this form to report, or apply for any of the following:

• moves or tenancy changes
• changes in owner/landlord or letting agent
• apply for reductions/reliefs
• request a refund
• advise us of a change in bank details
• any other change in circumstances

You will need to complete and submit this form for each property that you wish to notify us about.
